HEALTH SCREENING PACKAGE worth RM688.00 + RM300 CASH

Terms and Conditions:
1. Write a slogan in not more than 10 words either in Mandarin,
Bahasa Malaysia or English that captures the essence of Mahkota Medical
Centre. No limit on number of entry, but each entry must be original.
2. The entries shall become the proprietorship of Mahkota Medical Centre Sdn.
Bhd. and shall not be returned.
3. The top 15 slogans will be selected by Mahkota Medical Centre and the
decision is final. The winners will be notified and will be announced at
www.mahkotamedical.com.
4. The contest opens to all Malaysians above 18 years of age.
5. Employees and Doctors of Mahkota Medical Centre and their immediate
families are not eligible to participate in this contest.
6. By entering this contest, entrants shall accept these terms and
